Hi Cecilia,
I am going to give you a link to the Registration process and that pretty much explains everything. If you are missing the forms to fill out just email the Club office and they will mail them too you.

I needed some forms the other day and they came in two days. In addition to the registration forms you need a 4 generation pedigree, signed by the breeder and a Stud Cerficate signed by the owner of the Stud dog.
